• Short term care coordination (establishing a senior care plan to meet needs and working collaboratively with outside ​resources to make this happen)
  • Transitional care, from hospital to home (Ensuring that client has home health set up, durable medical equipment in home and medication is reviewed and organized, assist with obtaining caregiver in home if needed)
  • Home Safety Evaluation (Evaluation of home to help promote safety and prevent falls and accidents in the home)
  • Safety survey summary & recommendations (Client will receive a list of unsafe & potential hazardous risk factors and a recommendation of resolutions to help ensure safety, including fall preventive strategies)
  • Serves as a liaison with outside resources (will work collaboratively as a team with outside senior care resources) such as : In-home care providers, Home Health agencies, Outpatient therapy, Assisted Living communities, Alzheimer's care, Rehab, Skilled Nursing care, Hospice care, Durable medical equipment, Licensed contractors for installation of ramps & handrails, and Transportation set up
  • Medication review & organization (Organizing all medications in pill box on a weekly basis if needed, providing a typed list of all meds including times to be given and reason med was prescribed, checking for expired meds and calling in meds to pharmacy)
  • Organization of home (Organize common areas only to ensure safety and to make things more accessible for client)

Winter Safety Tips

  • Dress in layers and always wear a jacket when outside
  • Keep heater running
  • Be careful when using space heaters, they can be dangerous
  • When raining or snowing, wear shoes with good traction and non skid soles
  • Family check on your elder loved one often
  • Prepare for power outages; keep flash lights handy, have plenty of warm blankets in the home
